Lead care coordination and quality assurance for residents with dementia, physical limitations, and palliative needs in De Keyzer, Amsterdam. Coach colleagues on methodical, person-centered care while collaborating with treating professionals, family members, and neighbors to address early changes in care needs. Translate improvement opportunities into concrete team actions and maintain close proximity to residents' life stories. Work 28–36 hours weekly with a year-to-date contract, offering a salary up to €4,245.84 gross per month.
Required Qualifications
- Een afgeronde hbo-opleiding verpleegkunde
- Een geldige BIG-registratie
- Ervaring in de ouderenzorg of langdurige zorg
