Job Summary
Coordinate care activities and ensure high-quality somatic and psychosocial support for residents at HilverZorg locations. Perform nursing procedures, assess health conditions, and assist clients with daily tasks like bathing, dressing, and meal preparation. Facilitate multidisciplinary meetings, contribute to care plans, and manage risk-related interventions while maintaining optimal living environments. Develop effective communication skills to collaborate with families, caregivers, and colleagues. Work 24 to 32 hours per week including weekend shifts, with an indefinite-term contract and EVV training module available.
Required Qualifications
- Minimaal je diploma Verzorgende IG niveau 3 (of vergelijkbaar) in de pocket
- Je kunt een Verklaring Omtrent Gedrag aanleveren in de eerste werkmaand
- Interesse in en enthousiasme voor het ontwikkelen van coördinerende vaardigheden binnen de zorgsector
- Uitstekende communicatieve en organisatorische vaardigheden
- Bereidheid om te leren en te groeien in een dynamische zorgomgeving
- Teamspeler met een proactieve en positieve instelling
- Uitstekend inlevingsvermogen
- Goed in het opbouwen van banden met jouw cliënten
- Bereidheid om een EVV module te volgen (of EVV taken te hebben gekend tijdens opleiding)
