Job Summary
Coordinate placement and referrals for individuals with intellectual disabilities by actively approaching clients, managing waiting lists, and ensuring a minimum 98% bed occupancy. Process and submit timely requests for reindications and additional care while acting as the primary liaison between clients, families, care providers, and the care office. Provide advisory support to management on market trends and collaborate with the advisory team on care content. Maintain visibility within the organization by connecting with housing providers, colleagues, and external partners to identify opportunities. This interim role at Zideris, part of Zorggroep De Opbouw, offers a 36-hour workweek with hybrid working options. Candidates must hold an HBO diploma in care and welfare, possess a driver's license, and be available to start by mid-September.
Required Qualifications
- HBO diploma
- Experience in the role of zorgconsulent is a plus
- Possession of a driver's license
- Possession of a car
- Knowledge of the care world
- Familiarity with the Wlz and Wmo care laws
- Ability to apply Wlz and Wmo care laws in work
- Ability to start by mid-September
- Willingness to work in a temporary position of at least three months
- Willingness to work as a freelancer (ZZP'er) or via an agency
