Youth Tracker - PT B

The Youth Tracker is responsible for completing assigned daily contact with youth and meeting the conditions of the youth's service plan, with a flexible schedule that may include evenings, weekends and holidays. Responsibilities include conducting face-to-face daily contacts at various locations with youth, managing behaviors through programmatic and individual structure, developing a helping relationship emphasizing youth accountability, completing required documentation (including discharge information) for assigned youth, and entering data into the Juvenile Tracking System (JTS) and Efforts to Outcome (ETO). The role requires examining case files to stay current with youth history, notifying the Program Director of crises or incidents, maintaining regular attendance, assisting with coordination of youth activities, providing staff relief, offering youth advocacy in health, education, vocation, legal and family work, assisting with on-the-job training of new staff, providing crisis intervention and first aid when necessary, and adapting to change while performing other duties. The position involves daily travel (up to 85% to various GA counties) and uses the employee's own vehicle with travel reimbursement provided. Work shifts are subject to change and the environment includes varied locations with potential exposure to weather conditions.
