Job Summary

Conduct outreach to prospective service partners and follow up on referrals to engage youth and caregivers in the HOY program. Screen partners for eligibility and triage with Housing Navigators and Clinical Case Managers for needed services. Orient partners to TAY Academy services through intake and needs assessments, while providing crisis intervention, recreational opportunities, and peer support. Work collaboratively with wrap-around teams to accompany youth and caregivers to appointments, offering advocacy, independent living skills education, and referrals for housing, medical, educational, and mental health needs. Facilitate psycho-educational groups covering life skills, parenting, self-care, and health education to support reunification or transition to self-sufficiency. Maintain confidentiality, uphold SDYS standards, and communicate effectively across diverse backgrounds.

Required Qualifications

  • Must have direct experience as a consumer (or Care-Giver of a consumer) of mental health services, the juvenile justice system, child protective services, homelessness or special education
  • Must have directly experienced system involvement
  • Must be at least 21 years of age
  • Must have a High School diploma
  • Must have own, reliable transportation
  • Must be able to provide transport to youth as needed in the community
  • Must verify their eligibility to work in the United States

Desired Qualifications

  • Bachelor Degree Preferred
  • Bi-lingual desirable
  • Completed Peer Specialist training preferred
