Job Summary
Form and maintain relationships with youth participants, families, and support systems to help them set individual goals and create action plans. Work one-on-one to teach independent living skills, including budgeting, banking, and job readiness, while planning and supervising community activities like field trips and college visits. Transport clients to court appearances when necessary and connect families to appropriate community resources. Complete all documentation and data entry in a timely manner while complying with federal and state regulations. Maintain open communication with referral sources on a consistent basis.
Required Qualifications
- Bachelor's degree in Social Work, Criminal Justice, Drug & Alcohol Counseling, Human Services, Education or related field
- Valid NYS driver's license
- Must be able to lift, carry, climb, push, pull, balance, stoop, crouch, reach, handle, grasp, and feel
- Must be able to sit, stand, walk, talk, hear, and have good vision
- Must be able to perform repetitive motions and use eye/hand/foot coordination
- Must be able to work one on one or in a group, in the community or in the home
- Must be able to attend meetings and trainings in administrative buildings as required
- Must be able to transport individuals when necessary
- Must comply with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies, and all others
- Must be able to work 1:1 with youth to identify goals and create an action plan
- Must be able to assist youth participants with learning problem solving skills, improving communication and gaining independent living skills
- Must be able to plan and supervise youth in the community when attending field trips, workshops, college visits, etc.
- Must be able to form and maintain relationships with participants, their families and support systems, schools, community employers and partners
- Must be able to work with participants to teach independent living skills related to work and school
- Must be able to connect clients and their families to appropriate community resources
- Must be able to maintain open communication with the referral sources on a consistent basis
- Must be able to complete all documentation and data entry in a timely manner
- Must be able to develop, plan, and facilitate workshops
- Must be able to work with participants to develop positive work skills, explore colleges and careers, and to learn job readiness skills
- Must be able to teach participants how to function independently, such as teaching how to open a bank account and budgeting, interpersonal on intrapersonal skills
- Must be able to acquire safe housing, interpersonal on intrapersonal skills, and homelessness prevention
- Must be able to attend all court dates with clients, transporting clients to the court appearance if necessary
