Youth Service Coordinator

Conduct 90-60-30-day discharge meetings and support youth transitioning from residential placements back to the community by securing vital documents and opening bank accounts. Connect youth and families with community resources for housing, benefits, substance use support, tutoring, and mental health services while accompanying them for shopping and transporting them in program vehicles. Provide routine District Office coverage, conduct safety searches, monitor attendance, and track curfew or GPS requirements to ensure a secure environment. Enter client data into the JJEMS state database and manage tangible allowances. Work with DYS caseworkers and CHD staff to guide youth through reentry, focusing on positive youth development and strengths-based approaches.
