Job Summary
Youth Mental Health Case Manager provides evidence-based services to children and families to improve functioning in the community, including crisis intervention, skills and parent training, and coordination of medical, social, educational services. Responsible for recovery planning, routine assessments, service planning, Wraparound facilitation (if certified), and collaboration with community resources. Requires driving, field and office settings, and a bachelor’s degree in a related field with QMHP eligibility within 6 months; valid TX driver’s license, background check, and pre-employment drug screen. The role involves direct youth/family contact, risk assessment, crisis services, documentation, and adherence to HIPAA and program policies.
Required Qualifications
- Bachelor's degree in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology or early childhood intervention OR bachelor's with 30 hours in those fields
- Valid Texas Driver's license, auto liability insurance, and driving record acceptable to insurer
- Must pass criminal history and background checks; pre-employment drug screen
- Must obtain QMHP certification within 6 months of hire
- Driving may be required; travel to agency program sites, community and residential sites, and/or outside catchment area
- Flexibility in schedule; M-F 8am-5pm; possible after-hours travel
- Experience in mental health or social services preferred
- Knowledge of HIPAA and protecting confidentiality
- Ability to drive personal or company vehicle
