Job Summary
The Youth in Transition Outcomes Specialist assists the Case Manager in guiding older youth toward Single Case Plan goals and safe, timely case closure during the transition to adulthood. The role involves collaboration with the assigned case management team, older youth, biological families, resource families, external agencies, and partners. Focus areas include placement and in-home cases, direct work with families to address non-clinical supports, and coordination with partners to ensure youth receive community support necessary for reunification or other permanency.
Required Qualifications
- Bachelor’s degree in social work, social services, education, counseling, human services or related field required.
- A minimum of 2 years of experience working with older youth, young adults & families preferred.
- Must have strong interpersonal skills and the ability to communicate well orally and in writing.
- Must have a valid driver's license.
