Job Summary
Complete client intakes, assessments, and Person-Centered Plans, then submit initial and reauthorizations for Intensive In-Home services. Coordinate care with licensed therapists, families, schools, and community providers while meeting clients in their homes, schools, and community settings to deliver case management and skill-building. Monitor client progress against treatment goals, maintain accurate clinical documentation including NC TOPPS assessments and discharge records, and assist families in accessing community resources. Participate in Child and Family Team meetings as needed and manage assigned caseloads to ensure authorizations remain current. This contract role requires a Bachelor's degree, two years of behavioral health experience, and a valid North Carolina driver's license. Compensation is based on billable client contacts.
Required Qualifications
- Bachelor's degree in a Human Services-related field
- Minimum of two (2) years of documented experience working with children, adolescents, and families receiving behavioral health or mental health services
- Valid North Carolina driver's license
- Reliable transportation with current automobile insurance
- Ability to travel throughout Person County to provide services in clients' homes and community settings
- Ability to maintain a flexible schedule, including afternoon and evening appointments based on family needs
- Strong engagement and relationship-building skills with youth and families
- Excellent written, verbal, and interpersonal communication skills
- Knowledge of North Carolina Intensive In-Home service definitions and documentation requirements
- Strong organizational and time-management skills
- Ability to work independently while collaborating with a multidisciplinary treatment team
- Sound clinical judgment and effective problem-solving skills
- Proficiency with electronic health records and Microsoft Office applications
- Professionalism, flexibility, and a commitment to providing family-centered care
