Job Summary
Monitor cases and provide intensive community-based treatment for youth and families to promote family preservation and stabilize placement. Collaborate with therapists to plan discharge, coordinate resources including transportation, and attend family involvement meetings. Establish treatment programs by setting routines and arranging services, while making referrals to community providers and maintaining communication with all stakeholders. Respond promptly to client and family communications, verify attendance, and intervene in crises. Maintain client records, complete documentation within deadlines, and conduct weekly interdisciplinary evaluations. Prepare discharge plans, train family members, and update authorizations in the EMR and Optum systems. Comply with CARF, COMAR, HIPAA, and state regulations.
Required Qualifications
- Bachelor's degree in psychology, social work, counseling, or related discipline
- Knowledge of clinic territory
- Excellent communication and documentation skills
- Valid Driver's License and reliable transportation
- Ability to manage, delegate and manage multiple tasks with completing deadlines and priorities
- Excellent active listening skills
- Strong interpersonal and communication skills
- Excellent customer service and people skills are essential
- Responsible, reliable, and punctual
- Ability to be proactive and work in a fast-paced environment while ensuring clinical integrity and quality of services provided to clients and their families
- Ability to demonstrate understanding of managed care concepts and willingness to provide treatment and work within that system
- Strong organizational and prioritizing skills
- Ability to show sensitivity to the cultural and socioeconomic dynamics present among the practices service population(s)
Desired Qualifications
- Bilingual (Spanish) preferred
