Job Summary
Meet with youth clients 1–3 times monthly and family members at least once per month, while observing family visitations to assess progress toward permanency. Develop and implement individualized treatment plans by identifying service needs, strengths, and goals for each family. Coordinate wraparound community services and provide transportation for visits, medical appointments, and meetings as required. Collaborate with DCFS staff, legal teams, and community providers to ensure compliance and stability. Maintain accurate, timely case documentation. This role offers schedule flexibility and includes consistent supervision from a Child Welfare Supervisor to support professional growth.
Required Qualifications
- Bachelor's Degree in a Human Services–related field
- Valid Driver's License in good standing and active automobile insurance
- Ability to obtain an Illinois Driver's License within 30 days of hire if relocating from out of state
Desired Qualifications
- CWEL license preferred
