Youth Care Coordinator - 0007W - Mon-Fri 9AM-5PM Flexible

Establish supportive relationships with youth (ages 5–21) and families facing behavioral or physical health conditions to improve overall health and social functioning. Conduct outreach activities, perform comprehensive assessments including CANS-NY and trauma evaluations, and prepare care plans reflecting member needs. Maintain monthly contact via telephone or in-person visits to coordinate care, access resources, and reduce emergency visits. Collaborate with multi-disciplinary teams, attend weekly high-risk reviews and monthly management meetings, and conduct crisis interventions when necessary. Manage agency vehicles, track mileage, and complete required documentation for insurance continuity and billing. Provide consultation on medical and psychosocial issues to team members while reporting concerns to program administration.
