Job Summary
Youth Care Coordinator role within WellLife Network focusing on coordinating care for youth (ages 5–21) with significant behavioral and/or physical health conditions, conducting assessments (CANS-NY, Complex Trauma, Comprehensive Assessments), developing and revising care plans, maintaining monthly contact, linking to resources, coordinating with health home outreach, and supporting billing and documentation; requires a 4-year degree or RN license or Master’s with relevant experience, valid driver’s license, CANS-NY certification, and willingness to work in Suffolk/Nassau County, NY with duties including field and office-based activities and collaboration with a multidisciplinary team.
Required Qualifications
- Bachelors of Arts or Science Degree with two years of relevant experience (e.g. care coordination, youth and families)
- or a license as a Registered Nurse with two years of relevant experience
- or a Masters with one year experience
- Must maintain CANS-NY certification
- Must maintain valid driver’s license
