Job Summary
Mentor incarcerated young adult clients at the Rose M. Singer Center on Rikers Island for successful re-entry by reviewing records, conducting intake interviews, and creating personalized reentry preparation plans. Serve as a liaison between clients and community organizations to coordinate referrals for literacy, behavioral health, and housing needs while delivering small-group workshops on life skills and parenting. Assess treatment needs, develop trauma-informed plans, and monitor client progress through weekly interdisciplinary meetings and case note reviews. Advocate for entitlements, intervene in crises, and communicate with family members to support reunification and prenatal education. Work closely with counselors and DOC staff to coordinate discharge requirements and ensure clients navigate social services effectively.
Required Qualifications
- Lived experience with the criminal legal system
- BA in social work or other relevant degree
- 3 years' experience or 5+ years youth adult mentorship and case management experience
- experience with data entry
- proficiency with computers
- proficiency with Microsoft Office Suite: Word, Excel, PowerPoint, Outlook, and case management systems
- Ability to work independently
- be flexible and dependable
- demonstrate initiative and creativity
- successful completion and approval of a background check
- application to be conducted by the Department of Corrections & Community Supervision
Desired Qualifications
- Professional or personal experience providing mentorship to individuals with disabilities, mental illnesses, or challenging behaviors
- Bilingual in English and Spanish
- Previous experience working with young adults impacted by incarceration and reentry programming
- Proficiency in trauma-informed and/or motivational interviewing or similar person-centered engagement techniques
