Job Summary
Develop and maintain strong, trusting relationships with young adults transitioning out of foster care or experiencing unstable housing by conducting regular meetings in homes, community settings, or at the drop-in center. Collaborate with community partners to coordinate care and ensure access to necessary resources while teaching essential adulting skills such as budgeting and home maintenance. Maintain accurate documentation for tracking and reporting, and facilitate programs at the drop-in center to meet diverse needs. This role requires frequent travel within the community and working varied hours including evenings and weekends.
Required Qualifications
- Act 33/34 and FBI Clearances
- HS diploma/GED
- Valid PA driver's license
