Job Summary
The Wraparound Facilitator will work with families of children with serious emotional disturbances, providing intensive case management by facilitating a personalized planning process. Responsibilities include delivering services in various settings, accurately documenting services, coordinating Wraparound Team meetings, and ensuring compliance with applicable guidelines and regulations.
Required Qualifications
- Bachelor’s or Master’s Degree in human services related fields
- Must complete the Wrapround Training series
- Experience in the provision of home and community-based services
- CPR/First Aid Certification
- Behavior Management (MANDT) Certification
- Strong Verbal and Written Communication Skills
- Reliable transportation and current auto insurance coverage
Desired Qualifications
- Bi-Lingual/Spanish Speaking is a plus
