Wraparound Facilitator

Job Summary
Facilitate wraparound services for youth with severe emotional disturbance by developing integrated, family-driven care plans; coordinate medical, dental, vision, psychological, and academic or other specialized services; maintain an active caseload and complete progress notes and required reports; collaborate with MDHS/MDMH/MDOM and community partners; engage participants with the Wraparound Team to create an individualized, integrated care plan; work across office, family home, and community settings; maintain program records and meet licensing standards; require strong facilitation, communication, and multidisciplinary collaboration skills; schedule may include on-call duties, overnights, weekends, and holidays.
Required Qualifications
- Must be twenty-one (21) years of age.
- Must possess a minimum of a Bachelor’s Degree in a social services related field.
- Experience working with families and youth with emotional and behavioral needs preferred.
- Must have a minimum of one (1) year of experience in a child care program.
- Must be certified as a Community Support Specialist by DMH within 90 days of hire.
- Must attend and successfully complete a 3-day Wraparound Facilitator Training.
