Job Summary
Wraparound Facilitator responsible for coordinating community-based mental health and support services for youth and families within the CBS program. Build strong relationships with families, advocate across service systems, manage a caseload with significant emotional/behavioral needs, educate families about services including the HCBS SED Waiver, guide through referral-to-discharge processes, coordinate quarterly treatment planning meetings with families, providers, and insurance representatives, monitor progress with community partners, and ensure timely, accurate documentation meeting medical necessity and treatment goals. Requires a BA/BS or equivalent experience, independent work style with team collaboration, dependable vehicle, and ability to communicate effectively; direct client contact expected at a minimum of 60% of weekly time. Travel within community settings and coordination across schools, healthcare providers, and insurance entities expected.
Required Qualifications
- BA/BS degree or equivalently qualified by work experience in the human services field
- one year of experience substitution for one year of education
- valid driver’s license
- proof of insurance
- pass a criminal background check and driver’s license check
