Wraparound Facilitator

Job Summary
Wraparound Facilitator responsible for leading the treatment team through engagement and preparation, planning, implementation, and transition to ensure individualized care plans for youth and family, aligning services with National Wraparound Initiative standards. Responsibilities include engaging with youth and families, coordinating access to supports, conducting home/site visits, obtaining consents, completing CANS and other assessments, documenting activities per policy, developing an individualized care plan with the team, completing intake assessments for new clients, coordinating wraparound plan meetings, crisis response, advocacy at meetings and hearings, facilitating transition to least restrictive care, maintaining confidentiality and HIPAA compliance, and utilizing EMR systems for documentation. Requires knowledge of mental health in children, strong collaboration, flexible scheduling, and compliance with policies; validates English proficiency and driver’s license; CPR/First Aid and MANDT certifications preferred.
Required Qualifications
- Ability to perform Essential Duties as outlined below.
- BA/BS in Human Service Field.
- 1 to 2 years’ experience with Mental Illness & Behavioral Health Diagnoses.
- Possess knowledge of mental illness diagnoses and behavioral disorders in children.
- Flexible with schedule in order to meet with youth and family wraparound team members.
- Ability to collaborate closely with the youth's team and family members to develop and implement the individualized plan of care.
- Ability to comply with all of Valley’s Policies and procedures.
- Ability to comply with Consumer’s Rights.
- Ability to comply with Valley’s and Division departmental safety procedures.
- Ability to read, write and speak the English language.
- Valid Driver’s License.
- Ability to obtain and maintain CPR/First Aid, and MANDT Certifications.
Desired Qualifications
- Previous Experience in the MI & Behavioral Health Field for Children
- Previous Experience working in CSED, ACT, or other Wraparound Programs
- Previous Experience working in Service Coordination
