Job Summary
Wraparound Facilitator position responsible for developing wraparound plans with families, facilitating community-based wraparound team meetings, helping families access services, and coordinating a team of professionals and supports. Requires a BA/BS degree, at least 24 months of related experience (clinical, case management, skills building, child welfare, juvenile justice, or K-12 education), a valid driver’s license and reliable transportation, and ability to pass background screens. The role is full-time and hybrid, with travel to community meetings and the need to work from a home office within Indiana (near the state line).
Required Qualifications
