Job Summary
Provide intake services and information to families, referral sources, and community partners regarding the C.A.R.E.S. Wraparound model. Facilitate weekly Family Team meetings to develop individualized Plans of Care and Crisis Plans, while assessing strengths, needs, and resources to coordinate appropriate community supports. Offer supportive counseling and referrals to help children, youth, and families build on their natural supports. Maintain accurate documentation in CareLogic and complete required program data and reporting. Represent the program professionally in meetings and training, traveling as required to meet with families and coordinate services within the community. This role implements a strengths-based, family-driven approach to child abuse prevention and diversion.
Required Qualifications
- Candidates must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners
- Bachelor's degree in social work or related field
- one to two years related experience
- Must be at least 21 years of age
- Must successfully complete all agency background checks
- Requires active auto-insurance, a valid driver's license or the ability to obtain one with approved driver status
- Travel is required
- Must be able to lift 50 lbs
