Wound, Ostomy, and Continence Nurse (WOCN)

Job Summary
Conduct wound care consultations, oversee individualized care plans, and administer medications to clients in their homes. Perform comprehensive client assessments, deliver patient education to families, and monitor conditions from hospital discharge through independent living. Provide 1:1 skilled nursing interventions including wound care treatments and comfort care while advocating for client needs. Maintain an active PA RN license and WOCN certification to deliver compassionate, reliable care. Schedule includes Monday through Friday daytime hours with no weekends or holidays.
Required Qualifications
- Active and current license to practice as a professional registered nurse (RN) in the state of PA
- Wound, Ostomy and Continence Nurse (WOCN) certification
- At least one year of nursing experience
- Ability to travel to cases as assigned
- Monday - Friday position, NO Weekends and NO Holidays
