Wound Nurse

Job Summary
Wound Care Nurse provides specialized nursing care for residents with wounds in a long-term care setting. Collaborates with the interdisciplinary team including Director of Nursing, physicians, unit managers, and wound care coordinators to implement and evaluate wound prevention and treatment plans. Performs skin assessments (including Braden Scales), administers treatments, documents changes in skin integrity and care plans, leads staff education on wound care, and participates in committees and weekly rounds to optimize resident outcomes. Works with nursing, restorative, physical and occupational therapy to ensure proper pressure redistribution surfaces and device monitoring, and coordinates communication with residents and families about changes in skin condition and care plans.
Required Qualifications
- Graduate of accredited school of nursing
- Current, active license as a Registered Nurse in state of employment
- Wound Care Certification
- Active CPR Certification
- Three years of experience providing direct care to residents in a long-term care setting
