Wound Care Nurse LPN or RN

Job Summary
Assess, monitor, and document residents' wounds and skin conditions while providing wound care and treatments according to physician orders and established care plans. Monitor residents for changes in condition, notify the appropriate members of the care team, and maintain accurate clinical documentation. Collaborate with physicians, nursing leadership, and the interdisciplinary care team to assist with developing and updating individualized wound care plans. Educate residents, families, and staff on wound prevention and treatment while promoting infection prevention and ensuring compliance with facility policies. Provide compassionate, resident-centered care. Active North Carolina LPN or RN license required with wound care experience; long-term care experience preferred. Scheduling flexibility as needed.
