Wound Care Nurse (In-Home)

Job Summary
Assess and treat wounds in patients' homes using evidence-based guidelines and an application-based electronic patient record. Create individualized care plans, maintain accurate health records, and provide education on wound care techniques to patients and caregivers. Apply dressings and follow infection control protocols while prioritizing patient safety. Demonstrate strong communication skills to address concerns and ensure compliance with HIPAA regulations. Requires an active RN license, BSN preferred, and valid driver's license. Includes mileage reimbursement, health care plan, retirement plan, life insurance, paid time off, and training opportunities.
Required Qualifications
- Licensed Registered Nurse (RN) with an active and unrestricted nursing license in the state of practice
- Valid driver's license and reliable transportation, with ability to travel to patient homes
- Commitment to maintaining patient confidentiality and compliance with HIPAA regulations
Desired Qualifications
- Bachelor of Science in Nursing (BSN) degree preferred
- Minimum of three years of experience in wound care nursing preferred
- Certification in Wound, Ostomy, and Continence Nursing (WOCN) or other company-approved wound care certification is a plus; new hires may be eligible to take the certification exam within 60 days of employment with reimbursement upon passing
- Strong knowledge of evidence-based wound care practices and guidelines
- Proficient in documentation using electronic health record (EHR) systems
- Excellent critical thinking, multitasking, and communication skills
- Ability to work independently as well as collaboratively within a multidisciplinary team
