Wound Care Nurse (In-Home)

Job Summary
Deliver expert in-home wound care to patients in the comfort of their homes by assessing wounds, administering evidence-based treatments per WHG, and creating individualized care plans. Maintain thorough electronic health records, educate patients and caregivers on wound care techniques, follow infection control protocols, and collaborate with an interdisciplinary team while ensuring HIPAA compliance. Must travel to patient homes, demonstrate strong communication and customer service skills, and uphold patient safety and confidentiality.
Required Qualifications
- Licensed Registered Nurse (RN) with active and unrestricted nursing license in the state of practice
- Bachelor of Science in Nursing (BSN) degree preferred
- Minimum of three years of experience in wound care nursing preferred
- Certification in Wound, Ostomy, and Continence Nursing (WOCN) or other company-approved wound care certification is a plus (new hires may be eligible to take the certification exam within 60 days with reimbursement)
- Proficient in documentation using electronic health record (EHR) systems
- Valid driver’s license and reliable transportation, with ability to travel to patient homes
- Strong knowledge of evidence-based wound care practices and guidelines
- Excellent critical thinking, multitasking, and communication skills
- Ability to work independently as well as collaboratively within a multidisciplinary team
- Commitment to patient confidentiality and HIPAA compliance
