Job Summary
Provide diagnosis, treatment, and management of wounds in outpatient and home settings according to current standards of care. Educate patients and families on wound care, monitor progress, and adjust management as needed to achieve optimal healing. Collaborate with peers and PCPs, document thoroughly in EMR, and participate in ongoing education for staff and physicians. Adhere to evidence-based guidelines, utilize managed care processes for cost-effective care, and engage in organizational quality and care-delivery initiatives. Maintain licensure and certification requirements (AANP/ANCC/NCCPA) and DEA license as applicable, and contribute to patient education, care coordination, and program development within WellMed/OptumCare.
Required Qualifications
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Current Licensure through the Texas Board of Nurse Examiners OR Physician Assistant license by the Texas State Medical Board
- Valid DEA license or the ability to obtain one prior to employment
