Workforce Case Manager

Engage participants in an interactive screening process to identify needs, barriers, and goals; develop customized employment plans (CEPs) and Recovery to Work Plans (RTWs) to help job seekers overcome barriers and obtain placement in employment pathways. Conduct regular meetings to provide support and motivation, place job seekers in appropriate employment pathways according to aptitude, utilize workforce programs and financial support services to overcome barriers, ensure readiness for employment, input client data into the PDC case management system, and perform community outreach to inform participants and partners of available services. Role is based at a primary location with willingness to work out of alternate PDC offices (Youngstown/Salem/Warren).
