Job Summary
Assess, plan, and evaluate care for assigned patients by collecting data pre-, during-, and post-treatment, adjusting treatment plans, and documenting responses to intradialytic transfusions. Direct safe, effective patient care for modality-specific procedures while administering medications and initiating emergency response measures. Collaborate with direct care staff, physicians, and contracted facilities to ensure continuity, provide education to patients and families, and mentor new employees. Participate in on-call rotations for night, weekend, and holiday shifts, with occasional travel to training sites and outpatient facilities.
Required Qualifications
- Registered Nurse (RN)
- Inpatient RN CAP 2 designation or attained through advancement from CAP 1
- Accountable and responsible for the provision and coordination of clinically competent care
- Assessment, planning, intervention and evaluation for an assigned group of patients
- Delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians
- Participation in decision-making, teaching, leadership functions, and quality improvement activities
- Performance of all essential functions under the direction of the Supervisor and with the guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse
- Ongoing, systematic collection and analysis of patient data pre – during – post treatment
- Documentation in the patient medical record
- Adjustments or modifications to treatment plan
- Notification of Supervisor, Physician, patient's primary nurse and others
- Assessment, collaboration and documentation of patient/family's basic learning needs
- Initial and ongoing education to patients and family
- Recognition of aspects and implications of patient status that vary from normal
- Reporting to or collaboration with appropriate health team members
- Direction and provision of safe effective patient care for assigned patients
- Implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions
- Notification of adverse reactions to physician and appropriate blood supplier
- Administration of medications as prescribed
- Documentation of medical justification and effectiveness
- Initiation and assistance with emergency response measures
- Service as a resource, leader, coach, mentor and role model
- Setting examples of appropriate behavior, work habits and attitudes
- Assistance in an Outpatient facility on an as needed basis
- Heavy lifting, moving of patients and assisting with ambulation
- Movement of machines and equipment of up to 200 lbs.
- Lifting of chemical and water solutions of up to 30 lbs.
- Travel to training/meeting sites and between assigned facilities
- Participation in on-call rotation
- Night, weekend, holiday or as defined by individual program needs
- 6 months experience in acute dialysis as a RN
- Entry level for RNs with minimum 2 years or more of Nephrology Nursing experience in the last 2 years as a RN
- Drug-free workplace compliance

- Acute dialysis experience (preferred)
