Job Summary
Administer medications and treatments as prescribed while monitoring resident conditions and reporting changes. Provide direct patient care and support daily living needs, document care accurately, and support individualized care plans. Supervise and guide CNAs/caregivers as needed, ensure compliance with regulatory standards, and respond to emergencies to maintain a safe environment. Communicate effectively with residents, families, and care teams while performing other LVN duties as assigned. This role requires an active Texas LVN license, 6 months of experience in long-term care, and the ability to lift 50 lbs. Required Qualifications
- Active LVN license in the state of Texas
- Minimum of 6 months of experience in a long-term care, assisted living, or skilled nursing setting
- Strong clinical and communication skills

Desired Qualifications
