Job Summary
Independently licensed mental health therapists (LCSW, LMFT, or LPC) sought to provide diagnostic evaluations, psychotherapy, treatment planning, and consulting in-person at SNF/LTC facilities. Collaborate with BHS psychiatrists, psychologists, care coordinators, and facility staff for holistic patient care. Complete timely documentation in the EMR (Athena I). Ensure compliance with agency policies and procedures. Fluency in Spanish is a plus. Required Qualifications
- Master of Social Work, Counseling, or other eligible master's degree
- Colorado licensure as an LCSW, LMFT, or LPC
