Job Summary
Provide diagnostic evaluations, psychotherapy, treatment planning, and consulting in-person at SNF/LTC facilities; collaborate with BHS psychiatrists, psychologists, care coordinators, and facility staff for holistic patient care; complete timely documentation in our EMR (Athena I); comply with agency policies and procedures; qualifications include an MSW counseling degree and Colorado licensure as LCSW/LMFT/LPC; benefits include base salary with incentives, PTO, CEU support, and comprehensive health coverage.
Required Qualifications
- Master of Social Work, Counseling, or other eligible master’s degree and Colorado licensure as an LCSW, LMFT, or LPC
- NPI (National Provider Identifier)
- Credentialed with Medicaid/Medicare a PLUS
- TB Test
- CPR/AED/First Aid Certification
- Fluent in Spanish is a plus
