Job Summary
Provide health care and social services to pregnant women and families; educate women on self-care and parenting; offer emotional support; connect clients to needed services; manage a caseload of approximately 250 active clients; conduct outreach using case finding lists to identify eligible clients; bilingual English/Spanish preferred; requires Bachelor's degree in Social Work, Psychology or related field; background checks required; transportation reliability needed.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology or related field from an accredited college or university
- Minimum of one year of case management or counseling experience or counseling experience with children and families preferred
- Valid Illinois state criminal, child abuse, and FBI history required
- Proficiency in speaking English and Spanish preferred
- Reliable transportation required
