Job Summary
Conduct needs assessments with residents to translate their life histories and daily preferences into personalized care plans. Coordinate activities and day scheduling while collaborating with staff, volunteers, and family members. Coach care teams and volunteers through feedback and coaching conversations to foster self-management. Monitor daily functioning issues and lead meetings regarding well-being and day spending to drive quality improvements. Perform nursing care tasks and evaluate care plans as needed. Work in a flexible schedule aligned with resident and team requirements.
Required Qualifications
- Diploma minimaal MBO niveau 4 in bijvoorbeeld, Verpleegkunde, social work, SPH, CMV, MWD of event management
- Passie voor ouderenzorg
- Teamplayer
- Tactvol in je communicatie
- Begrijp je de zorgprocessen binnen de ouderenzorg
- Kun je goed inspelen op de behoeften van bewoners
- Betrouwbaar
- Geduldig
- Doorzettingsvermogen
