Job Summary
Conduct wellness assessments and direct services for individuals with serious mental illness enrolled in the PACT Program. Develop psychosocial recovery plans, monitor medication side effects, and coordinate care with community providers while linking recipients to housing, food, and entitlement benefits. Deliver skill-building training on hygiene, money management, and transportation, and provide rapid crisis response including psychiatric emergency center accompaniment. Maintain 24-hour rotating on-call coverage and participate in clinical supervision and team meetings. Work Monday through Friday with flexible scheduling and access to medical, dental, and vision benefits.
Required Qualifications
- Valid driver's license
- Vehicle required
- No more than one moving violation within the past 12 months
- A person who is receiving or has received mental health services may substitute demonstrated volunteer or paid experience working with individuals with serious and persistent mental illness in lieu of a Bachelor's degree
