Job Summary
Conduct comprehensive client assessments and develop individualized service plans for older adults across home, office, and community settings. Provide time-limited care management, screen for eligibility, and advocate to secure essential benefits such as Medicare and Medicaid. Direct-connect clients to transportation, homecare, meals, legal aid, and emergency medical or psychiatric care while completing required electronic case records and reports. Develop structured community outreach plans and represent the agency at public conferences and interagency meetings. Requires a Bachelor's degree in Social Work or Psychology, two years of human services experience, and bilingual Spanish fluency is preferred.
Required Qualifications
- Graduate of an accredited college or university with a Bachelor's degree in Social Work (BSW) or Psychology
- Minimum of two years of professional human services experience
- Ability to read, write, and speak English
- Computer literacy, including the ability to master program-specific record-keeping software
- Ability to travel to clients' homes and negotiate steps or physical barriers in various residences
- A deep understanding and appreciation of how race, religion, ethnicity, sexual orientation, and individual values shape service delivery for diverse families
