Job Summary
Provide on-site direct wellness and recovery-oriented services and case management to individuals experiencing mental health crises or worsening disorders. Engage clients using motivational and strengths-based approaches while conducting collaborative documentation and crisis de-escalation. Coordinate referrals to community service providers, assertively locating resources to divert individuals from emergency services. Assist enrollees in developing Wellness Recovery Action Plans and provide mental health education, illness management, and medication training. Maintain relationships with medical and mental health referral sources to fulfill case management needs identified in treatment plans.
Required Qualifications
- BA/BS/BSW in a social service discipline
- At least one year post degree work experience providing case management services for people with mental health and co-occurring disorders within the community setting
- Valid driver's license
- No more than one moving violation within the past 12 months
- Vehicle required
- Monday-Friday 9:00am-5:00pm (one evening shift required 12pm-8pm)
