Job Summary
Wellness Specialist provides comprehensive services within the ACT program, including assessment, counseling, and linkage to health, entitlement and vocational programs to support consumer placement and service plan development. Responsible for screening clients, coordinating case management, facilitating entitlements, acting as a clinical liaison, supervising and guiding the ACT team, developing treatment plans, collecting and analyzing data to evaluate performance and identify unmet needs, coordinating services with team members, facilitating consumer groups, ensuring timely documentation compliant with agency and OMH regulations, maintaining necessary driving capabilities for agency vehicle use, participating in trainings, offering crisis intervention, entering visits into Vellohealth, and providing 24/7 emergency coverage as needed. Requires strong communication, organization, and adherence to regulatory requirements and program policies.
Required Qualifications
- High school diploma or GED required.
- Bachelor's in human services preferred.
- Minimum of 2 years’ experience working with individuals with mental illness required.
- Two years experience working with Mental Health and Substance Abuse consumers preferred.
- Experience working with behavioral health population preferred.
- Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
- Valid New York State driver’s license required. Vehicle required for LI teams only– candidates are subject to regulatory background checks and must be willing to utilize own vehicle for home visits.
