Job Summary
Wellness Nurse RN responsible for monitoring the health and well‐being of residents and providing leadership support with resident care and services. Duties include coordinating health needs, conducting assessments at move-in and follow-ups, monthly wellness visits, documenting changes in health status, communicating with residents’ families and physicians, ensuring vital signs and weights are obtained, maintaining wellness files, and supporting infection control standards. Responsibilities also cover medication programs (transcribing orders, MAR/TAR audits, rights of medication pass), liaison with pharmacy services, and maintaining compliance with federal/state/provincial regulations. The role requires knowledge of quality improvement tools (APIE, QA indicators), participation in regulatory meetings, and ongoing training/compliance with Sunrise standards. Must be a graduate of an approved nursing program with current RN license, and able to work weekends/evenings; proficiency in computer skills and Sunrise applications is expected. Required Qualifications
- Graduate of approved college/school of nursing
- Maintains a current state/provincial license as a professional Registered Nurse (RN)
- Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
- Demonstrates knowledge of nursing practices, techniques and methods applied to health and wellness resident care, and federal, state/provincial, and local regulations
- Demonstrates knowledge of good assessment skills
- Demonstrates proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Sunrise applications with the ability to learn new applications
- Ability to work weekends, evenings, and flexible hours to be available to our customers at peak service delivery days and times
