Wellness Nurse - Memory Care

The Wellness Nurse (LVN) roles in memory care involve conducting resident health assessments and reassessments, managing medications, and coordinating with residents, families, and the care team to ensure high-quality, compliant care. Responsibilities include monitoring changes in condition, communicating with the Health and Wellness Director, assisting with hands-on care as needed, scheduling and assigning caregiving and med tech shifts, supervising and training care staff, transcribing physicians' orders, maintaining medical records and care plans, coordinating with home health agencies, addressing residents' needs and complaints, coordinating admissions with the sales team, and understanding state care regulations while ensuring seamless care delivery and teamwork.
