Wellness Nurse (LVN)

Administer medications safely and accurately per physician orders while overseeing medication aides and conducting monthly reconciliation audits. Coordinate daily staff assignments, provide direct care including ADLs such as bathing and mobility, and respond to medical emergencies with sound clinical judgment. Communicate effectively with physicians, families, and care teams regarding resident needs, complete clinical documentation, and monitor care team records for accuracy. Support orientation and training of care staff, attend required in-services, and perform other duties assigned to support resident care and operational excellence. Must work weekends and holidays.
