Wellness Nurse, LVN

Licensed Vocational or Registered Nurse (LVN/RN) coordinates resident care and medication management under the medical direction and supervision of residents' attending physicians to keep residents as independent as possible. Responsibilities include communicating with community healthcare professionals, coordinating various care services (medical, dental, vision, hearing, podiatry), administering medications and treatments as ordered, supporting Medication Technicians, assisting with resident assessments, conducting ongoing staff observations, documenting in resident charts, and coordinating prescription orders with doctors' offices and pharmacies. The role involves ensuring timely communication of resident health information to physicians and other healthcare professionals, and participating in shift-change briefings to relay pertinent resident care needs. A California nursing license in good standing is required; prefer 1 year of RCFE experience; must pass background checks, health screenings, and related regulatory requirements; ability to use MS Office and office equipment; willingness to comply with vaccination and PPE policies per regulatory guidelines.
