Job Summary
Wellness Nurse (LVN) coordinates resident care and medication management under the medical direction of attending physicians to maintain resident independence. Works on scheduled days off to provide seven-day coverage, communicates with physicians, discharge planners, nurses, and state agencies, coordinates incidental medical and therapy services, administers medications, supports Medication Technicians, assists with resident assessments, documents thoroughly, and coordinates with pharmacies and care programs to support residents’ health and well-being.
Required Qualifications
- Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing
- Prefer one (1) year of experience working in a Residential Care Facility for the Elderly (RCFE)
- Able to obtain and maintain valid first aid and CPR certification
- Criminal Background check and Health Screening tests (including TB) may be required
- Vaccination/testing/PPE requirements as per regulatory guidelines
