Job Summary
The Wellness Nurse (LVN) coordinates resident care and medication management under medical direction to maintain resident independence. Responsibilities include communicating with healthcare professionals (physicians, discharge planners, nurses, state agencies), coordinating incidental medical/dental/vision/hearing/podiatry care, administering medications per physician orders, supporting Medication Technicians, assisting with resident assessments, monitoring resident health changes with staff, documenting in resident charts, coordinating prescription orders with doctors and pharmacies, overseeing central medication storage and delivery, and communicating about preferred pharmacies and incontinence-supply programs. Must hold a valid California LVN/RN license in good standing and generally prefer RCFE experience. Proficiency with MS Office, ability to learn assisted living/dementia care concepts, and ability to work with seniors are required. CPR/First Aid certification and background checks are required; vaccination or PPE protocols may be requested.
Required Qualifications
- Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing
- Prefer one (1) year of experience working in a Residential Care Facility for the Elderly (RCFE)
- Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, etc.)
- Knowledge of and/or ability to learn the theory and practice of assisted living and dementia care
- Able to work with seniors and patiently interact with cognitively impaired individuals
- Able to obtain and maintain valid first aid and CPR certification
- Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
