Wellness Nurse, LVN

Wellness Nurse coordinates resident care and medication management under medical direction, supports Medication Technicians, conducts resident assessments, communicates with physicians and state agencies, and ensures consistent coverage seven days a week. Responsibilities include coordinating incidental medical, dental, vision, hearing, and podiatry care; administering medications and treatments as directed; documenting in resident charts; coordinating prescription orders with doctors and pharmacies; and maintaining communication with residents and responsible parties about care needs and preferred pharmacy/incontinence supply programs. Requires a California nursing license (LVN/LPVN) and ability to work in a residential care facility setting; willingness to pass background checks and health screenings, and to comply with PPE and vaccination guidelines as required.
