Wellness Nurse LPN - The Village at Ocotillo

Deliver nursing service duties in accordance with Healthcare Provider orders while overseeing resident care and the Medication Management Program. Participate in resident assessments to identify care needs, develop service plans with person-centered approaches, and follow up on health condition changes with providers and responsible parties. Perform nursing services, administer medications and treatments, and complete documentation via Yardi EHR. Collaborate with healthcare providers to review weekly notes, coordinate care for move-ins and extended leaves, and manage onsite lab testing and durable medical equipment. Support the Medication Management Program, assist with ADL care, and address daily staffing issues as needed.
