Job Summary
Meet with each resident regularly to provide health and wellness checks, answer health-related questions, and assess functional and psychosocial status. Initiate individualized service plans, oversee medication schedules, and audit medication records and narcotic counts twice weekly. Maintain ongoing communication with residents, families, and physicians regarding medical needs, coordinate additional health services, and ensure infection control guidelines are met. Perform personal care tasks including bathing, dressing, and feeding residents when necessary under the direction of the Wellness Director. Work as a Medication Care Manager on-call for staffing and scheduling, providing emergency coverage for medication assistance. Review changes in resident conditions with the Wellness Director and Executive Director.
Required Qualifications
- Current Nursing License issued by appropriate state licensing agency
- First Aid certificate
- Must have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals, etc.
- Must have good communication skills
- Must be able to effectively communicate in English
- Must show good judgement in recognizing symptoms indicative of resident's change in condition
- Must have a working knowledge of personal computing and word processing software
- Must be able to effectively communicate in English and understand and follow written and oral direction
Desired Qualifications
- Prefer a minimum of 2 years' experience working with the elderly in an assisted living, skilled nursing or hospital environment
