Job Summary
Wellness Nurse LPN/LVN at Sunrise of Metairie responsible for monitoring residents’ health and well-being, coordinating care with Resident Care Director and team, conducting monthly wellness visits, monitoring medications and documentation (MAR/TAR), ensuring infection control standards, contributing to quality assurance and regulatory compliance, and supporting training and development of staff. Requires a nursing degree/approved program, active LPN/LVN license, at least 1 year of relevant experience (home health, assisted living, hospital, or long-term care), strong communication and computer skills, and ability to work weekends/evenings with flexible hours.
Required Qualifications
- Graduate of approved college/school of nursing
- Current state/provincial license as LPN/LVN
- Minimum of one year experience in home health, assisted living, hospital, or long-term care
- Knowledge of nursing practices and regulations (federal, state/provincial, local)
- Good assessment skills
- Proficiency in computer skills (Microsoft Office, Sunrise applications)
- Ability to work weekends, evenings, and flexible hours
