Wellness Nurse

Conduct comprehensive health assessments for prospective and current residents, monitor changes in physical, cognitive, and emotional health, and initiate appropriate interventions. Develop individualized service plans based on resident needs and physician recommendations while coordinating physician appointments, laboratory services, therapy, hospice, and home health. Perform nursing assessments following falls, illness, hospitalization, or significant condition changes and respond to resident emergencies with appropriate interventions. Maintain accurate, confidential clinical documentation and resident records in accordance with HIPAA, ensuring survey-readiness for state inspections. Provide clinical guidance to associates, educate residents and families on health conditions and medications, and participate in interdisciplinary care conferences.
